Sir,

I read this case report with great interest. I am in complete agreement with the authors that the complications described are serious, but I beg to differ from the view that these are related to the procedure of toenail excision. Rather, I would blame the smoking in this case, which is very well known to cause problems with wound healing,1,2,3 so it would be appropriate to report it as complications of vascular disease or smoking, which would make it far less uncommon.
