Sir,

I read this paper with interest. We had treated a seven-year-old boy with bilateral clubfeet using a modification of this technique.1 We used short-leg casts which were changed weekly for six weeks. This was followed by Z-lengthening of the Achilles tendon and posterior releases of the ankle and subtalar joints for the residual equinus deformity. A short-leg cast was then used for two months followed by a custom-made ankle-foot orthosis.

We agree with Dr Lourenco and Dr Morcuende that the Ponseti method is feasible in the older child.
